I had a gay housemate in the early 80’s and he told me “when you’re gay, disease is a part of life.”
Fooling around butts sometimes is the problem, and playing with someone who has played with 1000 of them increases the risks.
Hepatitis and stds were typical. Later HIV was rampant.
So gay guys tend to have had frequent attacks on their immune systems.
