First, the pic in the OP is unflattering. She hasn't aged that poorly.. and it's not like Bill is exactly known for his good looks. Let's give credit where it's due. She's married to one of the richest guys in the world for almost 30 years and seemingly never had a nip or tuck. A more shallow person would be constantly upgrading their appearance. To her credit she didn't and seeming has let age take its course without enhancment.
